A lawsuit filed in Palm Beach County, Florida, alleges severe negligence and misconduct by a local rehabilitation center, resulting in the death of an elderly resident. The complaint outlines that Renee Hallak was a resident at Boca Raton Rehabilitation Center and suffered from various infirmities due to aging. According to the filing, FI-Boca Raton failed to provide adequate care and supervision for Renee Hallak during her stay. The plaintiff asserts that despite being aware of Renee's high risk for falls, the staff did not develop an adequate care plan or properly monitor her condition. This alleged negligence led to a catastrophic fall that necessitated emergency surgery and ultimately resulted in her death on May 23, 2023.

Daniel Hallak claims that FI-Boca Raton violated several provisions under Chapter §400 of Florida Statutes, which sets forth standards for operating nursing homes. Specifically, the defendant is accused of failing to provide appropriate healthcare and protective services, neglecting to implement necessary fall precautions, and delaying hospital transfer when Renee's condition deteriorated significantly. The lawsuit also mentions multiple failures such as inadequate staffing, improper training and supervision of staff, failure to respond timely to call bells, and lack of proper documentation regarding Renee's care.

The plaintiff seeks compensatory damages for various forms of suffering endured by Renee Hallak before her death. These include loss of dignity, pain and suffering, mental anguish, medical expenses, and funeral costs. Additionally, Daniel Hallak is pursuing compensation for the loss of companionship and protection experienced by Renee’s surviving children—Daniel Hallak himself along with Luisa Laui, Susanna Safdie, and Dianna Hallak.
