Provident Life and Accident Insurance Company accused of breaching contract

JACKSONVILLE — A man is suing Provident Life and Accident Insurance Company, alleging breach of contract.

Andy K. Zuttah filed a complaint on Nov. 3 in the Circuit Court for Duval County against Provident Life and Accident Insurance Company, alleging that the insurance firm wrongly denied his claim for disability benefits.

According to the complaint, the plaintiff alleges that in May 2015, he became disabled and applied for disability benefits but was denied. The plaintiff holds Provident Life and Accident Insurance Company responsible because the defendant allegedly refused to pay the plaintiff's disability benefits despite his disability under the terms of the plan.

The plaintiff seeks damages, all legal fees and interest plus any other relief as this court deems just. He is represented by Kimberly A. Corkill and Daniel M. Soloway of Soloway Law Firm in Pensacola.
