Phoenix Emergency Medicine of Broward seeks payment coverage from State Farm

ORLANDO – A medical facility claims an insurer failed to reimburse it for medical treatment.

Phoenix Emergency Medicine of Broward LLC, as assignee of Ralph Reichenbaum, filed a complaint on April 7 in the 9th Judicial Circuit Court of Florida - Orange County against State Farm Mutual Automobile Insurance Co. citing state statutes.

According to the complaint, the plaintiff alleges that on April, 13, 2012, Reichenbaum was injured in an accident and was provided care by Phoenix Emergency Medicine of Broward. The plaintiff holds State Farm Mutual Automobile Insurance Co. responsible because the defendant allegedly failed to submit payment for the personal injury protection benefits coverage within the 30 days receipt of the bill at issue and failed to pay plaintiff's covered losses in full.

The plaintiff seeks judgment against the defendant for personal injury protection coverage, medical payment coverage together with prejudgment interest, costs and attorneys' fees. It is represented by David B. Alexander of Bradford Cederberg PA in Orlando.
