CLEARWATER – A personal representative alleges an assisted living facility failed to provide her decedent a safe environment and failed to prevent her from falling.

Debbie A. Krieter, as personal representative of the estate of Dorothy Vandersalm, filed a complaint on June 1 in the 6th Judicial Circuit of Florida - Pinellas County against Nurses Helping Hands ALF Inc., Clementina Lazaro, Rolando Lazaro, Jason Lazaro and Jeanne Weiss alleging negligence and wrongful death.

According to the complaint, Vandersalm resided at Nurses Helping Hands from Jan. 1 to Jan. 26, 2016. The suit states Vandersalm died April 6, 2016. The plaintiff holds Nurses Helping Hands ALF Inc., Clementina Lazaro, Rolando Lazaro, Jason Lazaro and Weiss responsible because the defendant allegedly failed to properly train and supervise staff.

The plaintiff requests a trial by jury and seeks damages and all further relief as the court deems appropriate. She is represented by Jason R. Delgado of Wilkes & McHugh PA in St. Petersburg.
