Cheryl L. Howard filed a complaint on Jan. 23 in the U.S. District Court for the Northern District of
Florida, Pensacola Division
against Union Security Insurance Co. alleging violation of the Employee Retirement Income Security Act and breach of contract.
to the complaint, the plaintiff alleges that in 2014, she became
disabled due to kidney disease and other medial ailments and applied for
disability benefits. The suit states the benefits were initially
approved until her claim was terminated in 2016. The plaintiff appealed
the decision and the defendant has requested three extensions for a
review, but no determination has been reached, the suit states.
